Flactuation in TAC level and cretinine after 2 years of transplant

hello All, I am a 40 year Indian male , has undergone kidney tx two years ago. Last week my TAC was at 12.3 and Creatinine was 1.5 at prograf 0.5 BD . My doc reduced pangraf to 0.5 and 0.25 respectively. 0.25 in morning and 0.5 in evening. that reduced TAC to 4.95 and Creatinine to 1.5 in four days.I am having quite warm palm and feet since last few days and feeling heat in body. Can any body suggest what could be the reason of these fluctuation.

Hello Arpit,

Perhaps the most important issue a Tx  patient must stay aware of is hydration.  Proper hydration seems to cure many ills.  Conversely not drinking enough leads to dehydration, fever, high Creatinine levels and risk to your Tx.  Charting intake or drawing 1 to 3 liters(your Tx team will advise you on limits) of intake will be better than using memory.  I am glad that you contacted your team when you feel "off," I always suggest patients keep their Tx team aware  of any changes in how they feel, regardless of how long ago their Tx was.  I do not know what could be the cause of the feeling heat in the hands, perhaps it is medication related.  I wish great success with your Tx and managing the issue that arise.  Stay well!

Hi Arpit

Ask your Doc if you can switch to Advagraf which is a once a day medication .

Also every time you get a wonky result please recheck with the same lab. David is right .

How often do you check levels ?
